Solutions · health risk assessment

An employee health risk assessment people will actually trust.

Nine questions, answered by the employee. They get a private fitness score and tips. You get an aggregate risk picture, and never a single person's answers.

The problem

Challenges without a health baseline are half a story. You can see who is walking, but not who most needs a nudge, and employees will not hand that over unless privacy is obvious.

In one line

The health risk assessment is a short self-report questionnaire that scores an employee's health across nine everyday factors, returns a private fitness percentage with tips, and gives HR an aggregate, count-only risk picture, never an individual's answers.

Nine areas, answered by the employee

No lab test and no wearable. Employees answer what they already know, and can edit it any time from their profile.

Metabolic

Weighted 50%

BMI, waist, blood pressure and diabetes status.

Habits

Weighted 30%

Smoking and alcohol frequency.

Exercise

Weighted 20%

Activity frequency and fitness goal.

Card 1 of 3

Skip a question, no penalty

Missing factors are excluded and the rest re-weight, so a partial answer still gives a fair score.

The result is a fitness percentage from 0 to 100 with a status label and tips, and it is the employee's own number. Outputs are general wellness guidance, not a medical diagnosis.

Risk you can act on. Answers you cannot open.

Employees will only complete an assessment they trust, so HR visibility stops at the aggregate.

  • HR sees distributions

Org-level risk spread by area and a completion rate. Enough to design the right program.

  • HR never sees answers

No individual's questionnaire, score or status label is ever visible to an admin.

  • Target by risk, by count

Build a program for a health-risk cohort and you get the number of people, never a list of names.

Individual data is hosted in your chosen region — India, US, EU or UAE — and is never exposed to admins. Lab-report processing is HIPAA-guideline aligned.

A health risk assessment is a short self-report questionnaire that scores an employee's health across everyday factors like BMI, blood pressure, habits and exercise. In Vantage Fit it returns a private fitness percentage and tips, and personalizes the rest of the wellness program.
